Refund Policy

We understand that circumstances change. Our cancellation policy is as follows:

• •Consultation appointments must be cancelled at least 24 hours in advance to avoid a late cancellation fee
• •Surgical procedures must be cancelled at least 48 hours in advance; late cancellations of scheduled surgeries may result in a cancellation fee to cover facility and preparation costs
• •No-shows without advance notice may be charged the full consultation or pre-operative fee
• •Cancellations due to documented medical emergencies will be reviewed on a case-by-case basis

Patients are responsible for understanding their insurance benefits prior to receiving services. Soderstrom Surgery LLC will make reasonable efforts to verify your insurance coverage; however, insurance verification is not a guarantee of payment.

• •Any amounts not covered by insurance (deductibles, co-pays, co-insurance, non-covered services) remain the patient's financial responsibility
• •Patients without insurance coverage are responsible for payment arrangements prior to elective surgical procedures
• •We accept most major insurance plans and can discuss payment plans for qualifying patients

Refunds may be issued only in the following limited circumstances:

• •Billing errors: Charges applied in error that do not reflect services actually rendered
• •Duplicate charges: The same service billed and collected more than once
• •Overpayments: Patient payments that exceed the actual patient-responsibility balance after insurance processing
• •Prepaid surgery deposits for procedures that are subsequently cancelled by the practice

All refund requests must be submitted in writing and include:

• •Patient full name and date of birth
• •Date of service and procedure in question
• •Amount in dispute and reason for the refund request
• •Supporting documentation (receipts, Explanation of Benefits, etc.)
